Just looking around the BATF's label approval website and saw these two. Also Henry Weinhard's. Anybody seen them in real life?

Best to click on "Advanced Search" and highlight "Beer" "Ale" etc and then maybe narrow down by state.
On the basic search it doesn't always find stuff if you try typing in a brand name.
When it loads a list, click on the "TTB ID" number and it will load another page. best to right click and tell it "open in a new tab" so you don't lose your list.
Once that new page is open, click on "Printable Version" in the upper right of the COLA Detail box
That will open another window that shows the actual label approval form.
Scroll down to the bottom and an image of the actual label is there.
Unfortunately there is no way to tell if it's a bottle, can, barrel, 12, 16, 24, 40oz label until you go through that whole process.
